Respiratory System Information

 

Home oxygen

  • The Home Oxygen supplier for the South West Region is Air Liquide. Home oxygen should be ordered using the Home Oxygen Order Form (HOOF) which is accessed via the web base portal.
  • Prescribers will be required to complete the Initial Home Oxygen Risk Mitigation (IHORM) and consent form and store in the patient’s records.
  • The Part A portal allows non specialist staff to order static equipment (concentrators and static cylinders)  and will ask you to confirm that the patient has a signed consent form and that you have completed a risk assessment.   • The Part B portal is restricted to respiratory nurses and specialist teams.
  • If a patient requires ambulatory oxygen or a change in their ambulatory prescription, they will need to be referred to the local respiratory team. The Air Liquide HCP support team can be contacted on 0808 202 2099.
